Serve:One - 10 Year Plan

At His Branches, we seek to attack inequity and create hope as we join God on mission by emodying justice and mercy in our region. With this in mind, the theme for our 10 year plan is SERVE:ONE. Beginning with our current locations (19th Ward and Beechwood), we look to increase services and locations for greater access to quality and affordable health care so that, by 2032, we will have helped close the gap of life expectancy in our city by serving 1% of Greater Rochester’s residents toward their maximum health potential.


 Recent Partner Highlights

In collaboration with Ma’ama Tee’s Cookin’ & Catering and MVP Health Care, we’ve launched a prototype program in 2024 to address health care disparities by providing medically-tailored meals for minority patients at-risk for hypertension. Heart disease is the second leading cause of death for the Black community of Monroe County behind only cancer. The 12-week program is designed to demonstrate the impact nutrition, community, and medical care plays to create an opportunity for best possible outcomes.


In our sixth year of partnering with Bombas, we received 5000 socks and approximately 1100 tees and underwear to give to patients and neighbors in need in 2023. We assembled 100 Hope Bags filled with clothing, to be distributed by Ibero American Action League, to help and support climate refugees who have fled Puerto Rico to be with family members in Rochester following the effects of Hurricane Fiona this past September.


Two of the greatest barriers to healthcare are trust and transportation. We currently have 6 mobile technology stations to utilitize in strategic locations around Rochester, helping us further bridge the gap for people to access much needed medical & mental health care in places that have already established trust and safety. Our pilot telehealth hub launched at the YWCA of Rochester & Monroe County where residents can set up on-site televisits, eliminating the additional stress and challenges of transportation. We have since added two more hubs at St. Peter’s Kitchen and Delphi Rise in 2024.


We were honored once again to leverage investments from FLPPS to improve health access and outcomes through strategic programs and innovative technology within our service area. With their support, we’ve seen improvements in behavioral, maternal and child health outcomes, as well as a tightened referral system for supporting these individuals outside of our clinic. By providing internship opportunities, we are able to instill awareness and passion for neighborhood clinics in the next generation of the healthcare workforce. Since 2022, we’ve had ten interns complete a semester with us in a range of fields including nursing, pre-med, social work, and healthcare administration. We also have had multiple short-term projects for pharmacy and nursing students.

In June 2024, we are excited to begin a new partnership with Cedarville University as we welcome the first two students for a family medicine/women’s health care rotation here as part of their PA program.


Our partnership with Pirate Toy Fund allows us to continue forging connection and building trust with our youngest patients. We were able to give away over 800 toys and books from kids from birth to teens in 2023, giving them a sense of care and safety while encouraging healthy habits from a young age.

Part of our work in the neighborhood also involves pursuing economic equity and social stability in our community. We do this by sponsoring the Arnett Block Association and collaborating with strategic partners.

You can continue helping us make an impact in the communities we serve in a number of ways.

  1. Pray for our ministry, our team, and the individuals whose lives we get to impact on a daily basis.

  2. Give a one-time or recurring donation to help us go and grow. Donations help us cover gaps in reimbursement for medical expenses we experience as a safety net clinic, caring for everyone regardless of insurance or ability to pay. It also allows us to provide patient assistance on an as-needed basis for those in need, such as medical transportation, new parent baskets, or a hot meal for an individual in a crisis situation. Support is also needed for upcoming growth of our locations and staff.

  3. Purchase Neighborhood Blend Coffee from our ministry partner, New City Cafe (441 Ministries). A portion of every bag purchased of our custom blend is donated directly back to our ministry for patient assistance, continued operations, and greatest needs.
