His Branches operates as a community health center with clinics in the 19th Ward and Beechwood communities. We are accepting new patients and offer Primary Care with Integrated Behavioral Health, in addition to providing the following services:
General primary care for the entire family. This includes: annual checkups, sick visits, screening and laboratory work, preventative care, well-child services, immunizations and women’s health services.
Embedded within the primary care teams to ensure we address all of our patient’s physical, spiritual and emotional needs, patients meet with our
Behavioral Health Specialists
for an assessment and targeted intervention through mental health counseling as clinically necessary.
Our services include: routine GYN care, birth control methods, pregnancy testing and counseling, comprehensive prenatal care, including delivery with Rochester Regional Health Midwifery Department, voluntary family planning, testing and treatment for sexually transmitted infections.
Annual well-child checks, sick visits, and vaccinations for children ages birth to adulthood are available as part of our continuity of care model.
For quicker results and quality care, we offer the convenience of an on-site lab which allows us to check diabetes, blood pressure and cholesterol while patients are in the clinic for their visit.
Services include assistance with obtaining insurance and access to medical transportation, financial assistance for medical visits and medication copays, linking to Medicaid services, food and housing information/assistance for individuals/families experiencing homelessness, and access to mental health services.
Many appointments can be done remotely using an online video or phone conference method that provides an efficient way to attend appointments with our providers, from the comfort of your home or office. Telemedicine is appropriate for a wide range of services and you will receive the same quality of care as an in-person visit.
Initial medical screening for newly arriving refugees to enable successful resettlement. Additional primary care for ongoing health needs is also provided.
